==== Ketogenesis and ketoacidosis ==== Ketogenesis is a condition in which the liver releases ketone bodies, i.e., acetoacetic acid, beta-hydroxybutyric acid, and acetone, into the blood. This occurs when blood glucose levels are moderately low such as during sleep, fasting, dieting, pregnancy, and the first 28 days after birth (i.e., the neonatal period); this form of ketogenesis is associated with modest elevations in the blood levels of the ketone bodies and, due to their increased release from adipose tissue, fatty acids. The circulating ketone bodies and fatty acids serve as nutrients to sustain the functioning of critical organs such as muscle, heart, kidney and brain when blood glucose levels are too low to do so. During serious stress conditions such as diabetic ketoacidosis and non-diabetic ketoacidosis due to excessive alcohol intake, medications, toxins, or starvation (see ketogenesis sections on each of these conditions), blood glucose levels are very low, blood ketone bodies and fatty acid levels are very high, and (due to the high blood levels of the ketone bodies and fatty acids) the blood is extremely acidic. This condition, a form of acidosis termed ketoacidosis, is life-threatening. In addition to serving as a tissue nutrient and blood acidifier, one of the circulating ketone bodies appears to have another function: acetoacetic acid activates FFAR2.
The leaves of the coca plant contain alkaloids that—when extracted chemically—are the source for cocaine base. The amount of coca alkaloid in the raw leaves is small, however. A cup of coca tea prepared from one gram of coca leaves (the typical contents of a tea bag) contains approximately 4.2 mg of organic coca alkaloid. (In comparison, a typical dose (a "line") of cocaine contains between 20 and 30 milligrams.) Owing to the presence of these alkaloids, coca tea is a mild stimulant; its consumption may be compared to consumption of a moderately strong cup of coffee or tea. The coca alkaloid content of coca tea is such that the consumption of one cup of coca tea can cause a positive result on a drug test for cocaine, however. Similar to decaffeination in coffee, coca tea can be decocainized. Just as decaffeinated coffee retains a small quantity of caffeine, decocainized coca tea will still contain a small quantity of organic coca alkaloids. There is little information on the pharmacological and toxicological effects of consuming coca tea. A chemical analysis by solid-phase extraction and gas chromatography–mass spectrometry (SPE-GC/MS) of Peruvian and Bolivian tea bags indicated the presence of significant amounts of cocaine, the metabolite benzoylecgonine, ecgonine methyl ester and trans-cinnamoylcocaine in coca tea bags and coca tea. Urine specimens were also analyzed from an individual who consumed one cup of coca tea and it was determined that enough cocaine and cocaine-related metabolites were present to produce a positive drug test.
== Detection periods == The detection windows depend upon multiple factors: drug class, amount and frequency of use, metabolic rate, body mass, age, overall health, and urine pH. For ease of use, the detection times of metabolites have been incorporated into each parent drug. For example, heroin and cocaine can only be detected for a few hours after use, but their metabolites can be detected for several days in urine. The chart depicts the longer detection times of the metabolites. In the case of hair testing, the metabolites are permanently embedded into hair, and the detection time is determined by the length of the hair sample used in the analysis. The standard length of head hair used in the test is 1.5", which corresponds to about 3 months. Body/pubic hair grows slower, and the same 1.5" would result in a longer detection time. Oral fluid or saliva testing results for the most part mimic that of blood. The only exceptions are THC (tetrahydrocannabinol) and benzodiazepines. Oral fluid will likely detect THC from ingestion up to a maximum period of 6–12 hours. This continues to cause difficulty in oral fluid detection of THC and benzodiazepines. Breath air for the most part mimics blood tests as well. Due to the very low levels of substances in the breath air, liquid chromatography–mass spectrometry has to be used to analyze the sample according to a recent publication wherein 12 analytes were investigated. Rapid oral fluid products are not approved for use in workplace drug testing programs and are not FDA cleared.
Hence, in rats, vosilasarm is a potent full agonist of the levator ani muscle but a partial agonist and antagonist of the prostate and seminal vesicles, and is strongly selective for stimulating the levator ani muscle over the prostate gland. The lack of statistical significance was likely due to the small sample sizes per dosing group (n=3 each). No data on vosilasarm and lean body mass in humans have been published as of 2022.
